- For martyrs/injured police family: relationship verification documents
- For Janajati, Madhesi, Dalit, or backward regions: legal proof and certified documents as per guidelines
- All documents must be scanned and uploaded in PDF format
- Photo, signature, thumbprint, and both sides of citizenship must be uploaded in JPG format
